For good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, I, as the parent or legal guardian of the participant listed on this registration, a minor (hereinafter “Minor”), hereby grant the permission necessary to allow Minor to participate in Athletic and Cheering related activities (hereinafter “Activities”), to be conducted by Cheer Legacy Gym. I, in my own behalf and on behalf of Minor, further agree to release and to hold harmless CheerFactor on whose premises the activities will occur (hereinafter the “Location”) the affiliates of Cheer Legacy, CheerFactor and the Location, and the respective Owners, Representatives, Members, Agents and Employees of Cheer Legacy Gym or CheerFactor, the Location and their respective affiliates (hereinafter collectively “Releases’”) from any and all liability whether caused by the negligence of the Releases’ or otherwise for any claim, judgment, loss, liability, cost and expenses (including, without limitations, attorney’s fees and costs) arising out of or connected with the Activities, including any claim arising out of or connected with any illness or injury (minimal, serious, catastrophic and/or death) Minor may incur or sustain during the Activities and while traveling to and from the Location whether or not the Activities actually occur. I further expressly agree to indemnify and hold harmless Releases’ and Releases’’ heirs, successors, assigns, executors and administrators against loss from any further claims, demands or actions that may subsequently be brought by Minor or by any other persons on the account of damages of any character resulting to Minor in any way from the foregoing Activities.

I, in my own behalf and on behalf of Minor, hereby warrant that I have read this Liability Release in its entirety and fully understand its contents. I, in my own behalf and on behalf of Minor, am aware this Liability Release releases the Releases’ from liability and contains an acknowledgement of my voluntary and knowing assumption of the risk of injury or illness. I, in my own behalf and on behalf of Minor, further acknowledge that nothing in this Liability Release constitutes a guarantee that the Activities occur. I, in my own behalf and on behalf of Minor, have signed this document voluntarily and of my own free will.

Medical Release *

I, in my own behalf and on behalf of Minor, acknowledge and agree that such participation subjects Minor to possibility of physical illness or injury (minimal, serious, catastrophic and/or death) and that I, in my own behalf and on the behalf of Minor, acknowledge that Minor is assuming the risk of such illness or injury by participating in the Activities. In the event of such illness or injury, I authorize Cheer Legacy Gym to obtain necessary medical treatment of Minor and hereby, in my own behalf and behalf of Minor, release and hold harmless Releases’ in the exercises of this authority. I further acknowledge and understand that I will be responsible for any and all medical and related bills that may be incurred on behalf of Minor for any illness or injury that Minor may sustain during the Activities and while traveling to and from the Location.

Financial Obligation Agreement

By Checking the boxes I agree to the terms listed below. *All parents/guardians must understand that they assume full responsibility for all costs incurred as a memberof CHEER LEGACY CFAS TAUNTON including, but not limited to, gym registration/tryout fees, monthly tuition, practice outfits, uniforms, competition and travel expenses, camp fees, or any other item(s), reservations or services purchased by or rendered to Cheer Legacy Gym. The payment in full of all fees, regardless of any circumstances that may arise such as dismissal from the team, switching to a new team, etc., must be made promptly.All payment due dates must be met.Monthly payments are due the first week of each session.Tuition pays for training. It does not pay for the right to perform.Vacations or time off will not be prorated.All CHEER LEGACY CFAS TAUNTON families will be required to keep a credit or debit card on file with the office. Monthly payments later than the 15th of the month will be automatically charged to your credit/debit card on file. A$25.00 late fee is assessed if payment is not made by the 15th of each month.Any payments resulting in NSF (non-sufficient funds), declined credit card, expired credit card, returnedcheck and electronic debits, etc. will incur a $25 service charge. It is your responsibility to update your information with us if it changes (ex– bank acct number, card number, exp date, etc)All accounts must be in good standing to try out for the next season as well as continuing to participate in practices, competitions and other events.CHEER LEGACY CFAS TAUNTON reserves the right to sit an athlete out of practice or to remove an athlete from their teamat any time for failure to keep up with financial obligations.Any monies received for an overdue account will be applied to the athlete’s account items at the gym’s discretion.All tuition/fees must be current before an athlete collect any clothing/uniform or other retail items.Students with an outstanding balance may not schedule or participate in private lessons. They are alsoprohibited from taking classes or making pro-shop purchases until balance is brought up to date.If an athlete chooses to leave or is asked to leave Cheer Legacy Gym for any reason before the season is over, any and all funds are completely non-refundable.CHEER LEGACY CFAS TAUNTON reserves the right to turn over all delinquent accounts to collection agencies and the parent/guardian will be responsible for all additional costs incurred. Cheer Legacy Gym may alsopursue legal means to collect balances owed on any delinquent accounts.
